"LLL.  The Department of Medical Assistance Services shall develop and implement a  pilot program based on the Grand-Aides® program that uses mature nurse practitioners and physician extenders to work as part of a team, under the supervision of a nurse or physician to provide specific services in the community to Medicaid recipients, designed to reduce inappropriate Emergency Department visits by 25 percent.  The Department of Medical Assistance Services shall report the results of this pilot program, including cost and quality measures, to the General Assembly by December 2013".


Explanation
(This amendment adds language and provides $475,000 from the general fund each year to establish a pilot program based on a innovative model "Grand-Aides® program" that uses mature nurse practitioners and physician extenders to work with Medicaid recipients as part of a team under close supervision by a nurse practitioner or physician to reduce inappropriate Emergency Department visits. These aides use telephone protocols and home visits to provide primary care for simple conditions to prevent emergency department visits. They also provide chronic disease management and transitional services for individuals returning from the hospital to home. Language requires the Department of Medical Assistance Services to report the results of this pilot program, including cost and quality measures, to the General Assembly by December 2013.)
