Grants shall support community-based organizations for evidence-based food interventions, including produce prescriptions, medically tailored meals and groceries, nutrition education, and/or care coordination. Grants can also be used for capacity building, research and should align with the Rural Health Transformation Program."



Explanation

(This amendment provides $850,000 GF the first year for VDH to establish a Pilot Grant Program to develop a Food is Medicine model funding community-based interventions addressing nutrition-related chronic disease. Grants will support community-based organizations for evidence-based food interventions, including produce prescriptions, medically tailored meals and groceries, nutrition education, and/or care coordination. Theses funds can be used for food, capacity building, research and should align with the Rural Health Transformation Program.)
