"J. 1. Out of this appropriation, $865,000 the first year and $865,000 the second year from the general fund shall be awarded to the Virginia Health Workforce Development Authority (VHWDA) to establish a primary care practice transformation program within each Area Health Education Center. Funds shall be used to hire one practice transformation specialist within each of Virginia's eight Area Health Education Centers to facilitate primary care practice transition to team-based care models, and a practice transformation manager at VHWDA to provide program oversight.

2. The Virginia Health Workforce Development Authority shall develop needs-based criteria for program participation by primary care practices. VHWDA shall submit an interim progress report to the Chairs of the House Appropriations and Senate Finance and Appropriations Committees and the Joint Commission on Health Care by November 1, 2024, and a final report by November 1, 2025."



Explanation

(This amendment provides $865,000 each year from the general fund for the Virginia Health Workforce Development Authority (VHWDA) to establish a primary care practice transformation program within each Area Health Education Center, and hire additional staff. The VHWDA shall develop needs-based criteria for program participation by primary care practices and submit a report to the Chairs of the House Appropriations and Senate Finance and Appropriations Committees and the Joint Commission on Health Care (JCHC). The 2023 JCHC report on Team-based Approaches to Improve Health Outcomes found that team-based care implementation requires support such as onsite technical assistance and guidance on adjustments to health, but these supports are not always available to all primary care practices; independently owned primary care practices or those that do not meet the criteria for health plan incentive programs have limited access to business consultants; and several states have leveraged staff at Area Health Education Centers to support practice transformation in their regions.)
