(This amendment provides $75,000 GF each year to continue activities in the Rappahannock-Rapidan region for a model of long-term care system enhancement and service collaboration that could be used as a model for other localities in Virginia. Aging Together does not provide direct services but promotes long-term system improvements. This initiative began with a grant from the Robert Woods Johnson Foundation as a best practice.)
