"M.  Out of this appropriation, $1,000,000 the first year and $1,000,000 the second year from the general fund is provided to establish 10 grants of $100,000 each for projects that provide mobile, brief intervention and service linking as a form of care coordination to elderly individuals.  Projects shall replicate the Senior Outreach to Services (SOS) program currently administered by the Peninsula Agency on Aging.  A fifty percent local match is required to receive the grant funding."


Explanation
(This amendment provides funding to establish 10 grants for care coordination services, which are modeled on the existing Senior Outreach to Services (SOS) program. This program provides mobile, brief intervention and service linking to coordinate care for the elderly population it serves.)
